Acceptance of Terms
By accessing or using DocuSpa (operated by SKMD Wellness LLC, "DocuSpa," "we," "us," or "our") at docuspa.com, or by booking a Good Faith Exam through a partner medspa clinic, you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, please do not use our services. These Terms constitute a legally binding agreement between you and SKMD Wellness LLC.
Description of Services
DocuSpa provides a telehealth platform that facilitates Good Faith Examinations (GFEs) for patients of partner medspa clinics. Our services include:
- A secure online health intake and pre-assessment process
- Scheduling and conducting telehealth consultations with licensed healthcare providers (MDs, DOs, NPs, and PAs)
- Generating and delivering clearance documentation to partner clinics
- Account management tools for clinics and administrative users
- Transactional SMS and email notifications related to your appointments and account
DocuSpa does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.
SMS Program Terms
By providing your mobile phone number to DocuSpa, you agree to receive recurring transactional text messages including verification codes, appointment reminders, telehealth video visit links, and account notifications.

Telehealth Disclaimer
The Good Faith Exam conducted through DocuSpa is a telehealth consultation intended to evaluate your suitability for specific aesthetic or medical spa procedures. It is not a substitute for a comprehensive primary care visit, emergency care, or ongoing medical management of chronic conditions. The licensed provider conducting your exam will use professional judgment to assess the information you provide; the quality of that assessment depends in part on the accuracy and completeness of the information you submit.
- You are responsible for providing accurate and complete health information during intake.
- A clearance note issued by DocuSpa does not guarantee a specific treatment outcome.
- Some states have specific regulatory requirements for telehealth GFEs; our providers comply with applicable state laws.
- Your GFE provider may decline to issue a clearance note if, in their professional judgment, proceeding is not appropriate.
User Obligations
When using DocuSpa, you agree to:
- Provide accurate, current, and complete information during registration and health intake.
- Maintain the confidentiality of your account credentials and notify us promptly of any unauthorized access.
- Use the platform only for lawful purposes and in compliance with these Terms.
- Not attempt to reverse-engineer, scrape, or interfere with the platform or its underlying systems.
- Not impersonate any person or entity or misrepresent your affiliation with any person or entity.
- Not use the platform to transmit any unlawful, harmful, threatening, or abusive content.
Clinic Accounts
If you are accessing DocuSpa as a partner medspa clinic or administrative user, you additionally agree to:
- Ensure that patients referred to DocuSpa are informed about the telehealth exam process.
- Not share your clinic credentials with unauthorized individuals.
- Use clearance documentation only for the specific patients and procedures for which it was issued.
- Comply with all applicable federal and state healthcare regulations, including HIPAA.
Payment Terms
Fees for Good Faith Exam services are disclosed at the time of booking. Payment is processed securely through our third-party payment processor. Fees are non-refundable once a telehealth consultation has been completed. If a provider is unable to conduct the exam due to a technical issue on our end, we will reschedule at no additional charge. Clinics billed directly are subject to separate billing agreements.
Intellectual Property
All content, trademarks, logos, software, and platform functionality are the property of SKMD Wellness LLC or its licensors. You may not reproduce, distribute, or create derivative works from our content without our prior written consent. You retain ownership of health information you provide; you grant DocuSpa a limited license to use that information to deliver the services described in these Terms and our Privacy Policy.
Limitation of Liability
To the maximum extent permitted by applicable law, DocuSpa and SKMD Wellness LLC, including our officers, directors, employees, and agents, will not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services. Our total liability for any claim arising out of or relating to these Terms or our services will not exceed the amount you paid to DocuSpa in the twelve months preceding the claim.
Some jurisdictions do not allow the exclusion of certain warranties or limitation of liability, so the above limitations may not apply to you in full.
Disclaimers
DocuSpa is provided on an "as is" and "as available" basis. We make no warranties, express or implied, regarding the platform's uninterrupted availability, accuracy, or fitness for a particular purpose. We do not warrant that the platform will be free from errors, viruses, or other harmful components. Medical outcomes are not guaranteed and depend on many factors outside our control.
Indemnification
You agree to indemnify, defend, and hold harmless DocuSpa and SKMD Wellness LLC from and against any claims, damages, losses, liabilities, and expenses (including reasonable attorneys' fees) arising out of or related to: (a) your use of the platform; (b) your breach of these Terms; (c) your violation of any applicable law or third-party rights; or (d) inaccurate or incomplete information you provide during the intake process.
Governing Law
These Terms are governed by the laws of the state of Texas, without regard to its conflict of law provisions. Any dispute arising out of or relating to these Terms or your use of DocuSpa shall be resolved exclusively in the state or federal courts located in Texas, and you consent to personal jurisdiction in those courts.
Changes to These Terms
We may update these Terms from time to time. When we do, we will post the revised Terms on this page with an updated "Last updated" date and, where appropriate, notify you by email. Your continued use of DocuSpa after any changes constitutes your acceptance of the updated Terms.
